We are constantly striving to provide our patients with the best care possible for each individual situation. If you have been referred to our office by another physician, we will work out a plan for your care in coordination with the referring doctor.
Once you have made
your initial appointment, simply download the forms, complete, and bring the Patient Forms with you to your first
appointment. Check in with the receptionist upon your arrival.
What to bring with you on your first appointment:
- Insurance card(s)
- Patient forms. If this is your first visit to our office, please download our forms to fill out before your visit.

Please be advised that it is the patient's responsibility to obtain the necessary authorizations from their primary care physician or insurance company prior to being seen by one of our physicians. Authorizations vary with each insurance company, and we advise that you consult your insurance company or your primary care provider to determine if any pre-authorization or doctor referrals are required. If your insurance company requires an authorization, this authorization form or referral must be provided at the time of your visit to the doctor.
Payment Method and Deductibles
All office visit co-pays are due on the day of your appointment and may be made by cash or check.
Our office sets aside time for each patient in order to provide the best care. We understand that it may be necessary to reschedule an appointment, so we request that you contact our office at least 24 hours prior to your scheduled appointment. An appointment No-Show will be charged $25.00.
